Working as part of an interdisciplinary team, the Weller Clinic’s Diabetes Educator accepts internal referrals from KCHC Weller Clinic’s rostered patients living with Type 1 and Type 2 diabetes mellitus. Our Diabetes Educator works collaboratively with clients and their families to teach diabetes self-management, promote healthy lifestyle choices, and help create individualized treatment plans that align with personal goals.
Topics covered can include:
- Identifying blood glucose trends
- Personalizing diabetes management strategies and self-care
- Lifestyle and nutritional counseling
- Basic to advanced carbohydrate counting
- Medication
- Initiating insulin and safe dose adjustments
- Hyperglycemia /hypoglycemia signs & symptoms and treatment
- Foot screenings
- Diabetes and driving
- Connecting clients to internal or community supports to optimize good health
