The Online Self-Management Program

In partnership with our sister programs across Ontario, we also offer free online workshops and coaching to individuals who are living with one or more chronic conditions such as diabetes, chronic pain, high blood pressure, stroke, cancer, heart, or kidney conditions. Many of our workshops are also beneficial for caregivers and anyone looking to improve their health literacy.

These online workshops cover both similar and different content as our in-person workshops; this means that they are great to take in addition to, or instead of, those workshops. Help With Your Health

Help With Your Health is a five-week interactive webinar series for those wanting to manage their health to the best of their ability. Each week you will make a plan that supports healthy changes that are important to you.

The workshop is designed to be practical, enjoyable, non-judgmental, and accessible to a wide range of participants. The group will be sharing experiences and learning together; it is not about testing knowledge.

Health Coaching

Individuals living with chronic health conditions spend very little time with health professionals, most of the time they are managing on their own.

Health Coaching is a free one-on-one web-based support program to help you manage your conditions and get the most out of life. 

A coach provides support and guidance to help you make the healthy changes you want to work on such as managing your health condition(s) and its complications, finding helpful community resources, and taking action to live a healthy life. Their expertise stems from their shared experience with chronic conditions.

Living Healthy Webinars

Webinars are free and easy to access interactive sessions that are between 30 and 60 minutes in length and usually feature an expert guest speaker. Each webinar will explore different aspects of managing our health with helpful information and tools you can use in your daily life. You will have a chance, in most instances, to interact with the guest speaker and ask them questions.
