The Chronic Disease Self-Management Program (CDSMP) is a 6-week online workshop, meeting twice a week for one hour on Tuesdays and Thursdays from 12:00 PM to 1:00 PM EST, starting January 28, 2025 and ending March 6, 2025.

Please note that being at a workplace is not required nor is having any kind of work/job – the name of the workshop is due simply that it takes place over lunch hours. You are very welcome to join from home or anywhere!

This FREE program is designed for adults living with one or more chronic conditions, such as diabetes, heart disease, lung disease, cancer, depression, and more. It also welcomes individuals at risk for chronic diseases, as well as family members and caregivers.

Led by two trained facilitators—volunteers who also live with chronic conditions—the program offers practical tools to manage symptoms like fatigue, pain, sleep problems, shortness of breath, stress, and emotional challenges such as anger, fear, and frustration. Participants also learn techniques to balance work, home life, and improve overall well-being.

This program is based on evidence-based practices developed by Stanford University and is supported by the Self-Management Resource Center. All materials have been reviewed by health professionals, including physicians, dietitians, and therapists.
