This FREE program is designed for adults living with one or more chronic conditions, such as diabetes, heart disease, lung disease, cancer, depression, and more. It also welcomes individuals at risk for chronic diseases, as well as family members and caregivers. Led by two trained facilitators—volunteers who also live with chronic conditions—the program offers practical tools to manage symptoms like fatigue, pain, sleep problems, shortness of breath, stress, and emotional challenges such as anger, fear, and frustration. Participants also learn techniques to balance work, home life, and improve overall well-being.
