Brief Action Planning is a highly structured, patient-centered and evidence-informed model based on the principles and practice of Motivational Interviewing (MI). It is a structured way of having a conversation with someone who is interested in turning their hopes for healthy behaviours into concrete plans. The core skills can be used by anyone interested in supporting others to make change.
Brief Action Planning (BAP) addresses many of the barriers’ providers have cited to providing self-management support. It can be used routinely by both individual providers and health care teams to facilitate patient-centered goal setting, action planning and to build self-efficacy in chronic disease management and prevention.
