Position Summary
The Community Liaison will play a key role in connecting clients, health care providers, and community partners to the Regional Self-Management Program (RSMP) and other Kingston Community Health Centres (KCHC) initiatives. This position supports individuals living with long-term health conditions by promoting access to workshops, , and community resources that help address unmet health needs. The position also supports health care providers in accessing professional development opportunities
The Community Liaison will provide administrative and outreach support across KCHC programs. They will promote learning opportunities to diverse audiences, helping clients, families, and providers navigate and access the supports available through the Self-Management Program in South-Eastern Ontario, in alignment with KCHC’s agreement with Ontario Health at Home. Building and maintaining relationships with health care teams, family health organizations, and community-based agencies will be central to ensuring that providers and clients are aware of and connected to available programs and resources.
