This program offers practical solutions, information referral, and connects individuals and families with appropriate resources within the community. Rooted in establishing connection between the client and supports to help address unmet social health needs, this program offers assistance around housing, income security, food security, transportation, literacy, and social isolation.
We are part of a multi-site health collective that uses a common electronic medical record system along with Greater Napanee Health Home, Midtown Kingston Health Home, rural Kingston Family Health Organizations (Sydenham, Tamworth, Newburgh & Verona clinics) as well as some specialist physician partners with whom we collaborate. If your primary care provider works in any of these clinics they will be notified of your visit at KCHC.
