Join the Self-Management Program of Southeastern Ontario for this free 6-week workshop that helps you actively achieve your best health while living with a chronic condition.

The Living Well with Chronic Conditions: Lunch Hour program meets online twice a week for 6 weeks (a total of 12 sessions). It will be taking place on Tuesday and Thursday afternoons from 12:00pm to 1:00pm EST, from Tuesday September 29, 2026 through Thursday November 5, 2026.

Due to funding regulations, we are only able to accept participants from Southeast Ontario.

The Living Well with Chronic Conditions: Lunch Hour program provides information and skills to those living with one or more chronic conditions, typically while maintaining a job, to help them better manage their condition and achieve their best health.

You will learn techniques to deal with symptoms of chronic conditions, such as fatigue, pain, sleeplessness, shortness of breath, stress, and emotional problems such as depression, anger, fear, and frustration.

This free workshop is for adults of all ages, living in Southeast Ontario, who have any type of chronic diseases such as diabetes, heart disease, lung disease, cancer, and depression, among others. This workshop is also open to those at risk of developing a chronic disease, as well as their family members and caregivers.

The workshop is led by two highly trained leaders who also live with chronic conditions themselves.

Participants report improvement in their lifestyle and general health, as well as improved symptom management and better communication with their doctors. You will receive a copy of the book Living a Healthy Life with Chronic Conditions.

This workshop is an evidence-based self-management program originally developed at Stanford University and currently supported by the Self-Management Resource Center. Physicians, registered dietitians, occupational and physical therapists, and other health professionals have reviewed all workshop materials.

Topics include:

  • Techniques to help balance work and home life
  • Techniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Decision-making
  • How to evaluate new treatments
