Nurse Practitioner – Street Health Centre

Position Summary

The Nurse Practitioner is a member of the primary health care team. The emphasis is on holistic care, health promotion, disease prevention and includes addictions health care. Specific positions available supporting the RAAM clinic (rapid access addiction medicine) at Street Health Centre, as well as primary care. The Nurse Practitioner works as an integral part of clients’ broader healthcare teams and in close collaboration with an MD and a multidisciplinary allied health team.  The Nurse Practitioner is responsible and accountable for their practice.
