Our Diabetes Educator works with clients and their families to teach diabetes self-management, promote healthy lifestyle choices, and help create individualized treatment plans that align with personal goals.
Developed by universities, the Diabetes Self-Management Program runs for six weeks for 2.5 hours/week, and gives you knowledge and skills to put your diabetes care plan into action.
Our registered practical nurse offers foot care to Kingston area residents for significant health problems.
The Greater Napanee Health Home provides access to team-based primary care at multiple sites, including support from physician specialists, nurses, allied health professionals and administrative staff.
If you cannot access a dietitian through your family health team, you may self-refer or ask your health care practitioner for a referral. Clients and participants must live in the Kingston area or be a client of Kingston Community Health Centres.
Primary health care services are available at 263 Weller Ave. We are currently at capacity, but will take on unattached patients who are pregnant (for prenatal care). We are currently unable to take on any other patients, including children under five years of age.
