The Greater Napanee Health Home (GNHH) provides access to team-based primary care at multiple sites in the Greater Napanee area. Physicians and nurse practitioners are supported by nurses, an Allied Health team and physician specialists – this allows all providers to work to their full scope of practice while addressing the care needs of the population.
Health Homes are the new model of care introduced by the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT). The Mighty Docs Family Health Organization in collaboration with the Kingston Community Health Centres (KCHC), Lennox & Addington County General Hospital and the Town of Greater Napanee launched the Greater Napanee Health Home in April 2024.

Programs and Services at this Location
The Greater Napanee Health Home provides access to team-based primary care at multiple sites, including support from physician specialists, nurses, allied health professionals and administrative staff.
Napanee Area Community Health Centre serves about 60,000 residents of the Rural Frontenac Lennox and Addington region, spread out over a very large and rural district. Many programs and services are offered at this Napanee site.
The Access Clinic at the NACHC site offers primary care for urgent health issues for people who have already been rostered to the GNHH but have not had their first appointment with their primary care provider.
Counselling and therapy services are offered to individuals who are patients, clients, or participants in KCHC programs.
We provide oral health services to low-income individuals and families with limited or no access to dental care.
Certified Diabetes Educators offer programs and services out of our Napanee office to help people manage their diabetes.
The NACHC offers free sterile one-time use supplies for people who inject or inhale drugs. Accessing supplies is kept confidential.
Our kinesiologist works with individuals and groups. She will assess physical health goals, then provides an exercise prescription and follow-up to support implementation.
Registered Dietitians help people understand the connection between their health and their food choices. Our dietitian offers individual nutrition counselling and runs some group programs
Practical Assistance and Community Outreach Workers help people and their families to meet their practical needs, focusing on the social determinants of health,
This program supports patients and families with respiratory assessments and provides information regarding lung health.
Sexual health services specialize in sexual and reproductive health, such as birth control, pregnancy tests, sexual health education, and more.
The Smoking Treatment for Ontario Patients (STOP) program offers counselling support and Nicotine Replacement Therapy (NRT) supplies at no cost to patients wishing to quit smoking cigarettes.
The Lennox & Addington Youth Services is based out of the Napanee Area Community Health Centre and offers services and programs to youth aged 10 to 25 and their families.
