Bridge Family Health site of the Greater Napanee Health Home offers ongoing comprehensive primary care through a team-based model. Family physicians are supported by nurses, a team of allied health professionals and specialist physicians. Together the dedicated team supports people’s best health and wellness through every stage of their lives.
The Health Home model of care is being introduced by the Frontenac, Lennox & Addington Ontario Health Team (FLA OHT) across the region. A Health Home is a home base for easy access to all the health care and wellness services needed to achieve our best health.

People without a primary care provider (family physician or nurse practitioner) can receive care at the GNHH Access Clinic, while waiting to get attached.h different hours to serve the community better – one in Napanee and one in Deseronto.
People need to be rostered to the clinic’s Family Physicians to receive care at this site.
People could get rostered the Greater Napanee Health Home if they live in the area and do not have a primary care provider (family physician or nurse practitioner), by registering with Health Care Connect. Learn more about Health Care Connect.
