Midtown Kingston Health Home is a site of Kingston Community Health Centres, offering access to team-based care!
We are currently able to take patients at our 791 Princess St. location and do so by working with Health Care Connect. Those without a primary care provider are encouraged to register through Health Care Connect by phone at 1-800-445-1822 or online.
As we develop our patient-base, we continue to offer unattached care so individuals in the community can access primary care.
Please note that this location is temporary and will move to our permanent home in 2026.
What we offer
Our primary health care team includes:
- Nurses, nurse practitioners and physicians
- A certified diabetes educator
- A family health educator
- Social worker and psychotherapist
Medical students, nursing students, and resident physicians also work at Midtown Kingston Health Home at times throughout the year. They are supervised by our nurse practitioners and/or physicians.

Please note: Appointments available through the Midtown Kingston Health Home are intended for individuals that currently do not have a family doctor or do not have access to medical service providers. If you are a Queen’s University, St. Lawrence College or Royal Military College student, please consult your institution’s student health services (e.g., Queen’s Student Wellness Services, St. Lawrence College Kingston Campus Health Centre).
We are part of a multi-site health collective that uses a common electronic medical record system along with Greater Napanee Health Home, Kingston Community Health Centres, rural Kingston Family Health Organizations (Sydenham, Tamworth, Newburgh & Verona clinics) as well as some specialist physician partners with whom we collaborate.

Please note that our phones are answered on a slightly different schedule than our open hours. Phones are answered from 9 a.m. to noon and 1 to 4 p.m. on Mondays, Tuesdays, Wednesdays, and Fridays, and from 1 to 7:30 p.m. on Thursdays.
Our location
Midtown Kingston Health Home is located at 791 Princess Street, Suite 201. It is on the second floor and must be accessed through the elevator.
How to get to us? There are a few bus routes to choose from, including routes 4, 501 & 502. The clinic is about 900 metres from the Kingston Centre (or about a 15 minute walk).
You can come by car and park on site. Parking is available through the Grosline Lot, operated by the City of Kingston. It is accessible from the intersection of MacDonnell and Princess Streets. Payment is required from Monday to Friday, from 8 a.m. to 5 p.m. Parking costs $1.50 per hour.
We are not easy to find as we are hidden in a commercial building, adjacent and attached to other commercial buildings. Please look for the blue and white canopy at the front or for the Midtown Kingston Health Home sign if coming by the back (through the parking lot).
