Midtown Kingston Health Home is a site of Kingston Community Health Centres, offering access to team-based care!
We are currently able to take patients at our new location at 2-315 Queen Mary Road and do so by working with Health Care Connect. Those without a primary care provider are encouraged to register through Health Care Connect by phone at 1-800-445-1822 or online.

Our primary health care team includes clinicians and Allied Health professionals:
- Nurses, nurse practitioners and physicians
- A diabetes educator
- A family health educator
- Social workers and mental health counsellors
- A practical assistance worker
- A social prescribing worker
- A preventive care nurse
- A smoking cessation nurse

Medical students, nursing students, and resident physicians also work at Midtown Kingston Health Home at times throughout the year. They are supervised by our nurse practitioners and/or physicians.
We offer low barrier and judgment-free services to the community (including those who are not registered with us). These services include:
- Sexual Health
- Quick Test Clinic
- Prenatal Care
- Preventive Care Screening
- IUD clinics
- Pap tests
- Mammograms
- Colon screening
- Lung screening
- Family health education
We also offer interpretation services as needed.
We are part of a multi-site health collective that uses a common electronic medical record system along with Greater Napanee Health Home, Kingston Community Health Centres, rural Kingston Family Health Organizations (Sydenham, Tamworth, Newburgh & Verona clinics) as well as some specialist physician partners with whom we collaborate.

Please note that our phones are answered on a slightly different schedule than our open hours. Phones are answered from 9 a.m. to noon and 1 to 4 p.m. on Mondays, Tuesdays, Wednesdays, and Fridays, and from 1 to 7:30 p.m. on Thursdays.

How to get to us
By Kingston Transit
Midtown Kingston Health Home is conveniently located on Queen Mary Road. If you are travelling from Downtown Kingston (City Centre), you can take Route 32 with direct service from Downtown to Queen Mary Road. The clinic is a short walk from the nearest bus stop on Queen Mary Road.
By car
Free on-site parking is available for patients visiting the clinic.
