Kingston Community Health Centres' newcomer bridging clinic is dedicated health service for refugees, protected persons, and other newcomers who are new to Canada and face language or system barriers when trying to access health care. This clinic offers transitional primary care for up to two years, giving clients the time and support they need to settle, build trust, and eventually connect with a long-term family doctor or health care provider.
The Greater Napanee Health Home provides access to team-based primary care at multiple sites, including support from physician specialists, nurses, allied health professionals and administrative staff.
This Health Home provides primary care and serves as an important community access point for delivering population health services for the broader community, including cancer screenings, prenatal care, well baby care, and sexual health services.
Through our prenatal care program, we help connect clients to a physician, nurse practitioner, obstetrician or midwife.
