The Greater Napanee Health Home (GNHH) launched in April 2024 – through a collaboration with the Kingston Community Health Centres, the Mighty Docs Family Health Organization, the Lennox & Addington County General Hospital and the Town of Greater Napanee.
Health Homes are the new model of care introduced by the Frontenac, Lennox and Addington Ontario Health Team, in which comprehensive ongoing primary care is provided through a team-based model.
This GNHH provides team-based care at multiple sites in the Greater Napanee area.
What is team-based care?
Primary care providers (physicians and nurse practitioners) are supported by nurses, allied health providers and physician specialists. The GNHH team:

- Social workers & Psychotherapists
- Respiratory therapist
- Kinesiologist
- Practical assistance worker
Together this team can deliver care according to their full scope of practice while addressing the care needs of the population.

GNHH Access Clinic
This clinic is for people who have already been rostered (attached) to the GNHH, but have not yet had their first appointment – “Meet and Greet” with the new primary care provider (family doctor or nurse practitioner)
- Appointments can be made for:
- prescription renewals (not for controlled substances)
- acute episodic or urgent (non life-threatening issues) issues, such as:
- Urinary tract infections (UTIs)
- ear or eye infections
- animal or insect bites
- new rashes

How can I receive team-based care at GNHH?
If you do not have a family doctor, you first need to register with Health Care Connect (the provincial system for rostering to a doctor or nurse practitioner) by:
- Calling 811 or 1-800-445-1822 – to speak to a nurse who will ask a short series of questions, OR
- Registering online at Health Care Connect – to enter your information online quickly and easily.
Health Care Connect will attach you to this Health Home based on your address in the Town of Greater Napanee. This Health Home has a growing team of primary care providers – as capacity increases, more people will be attached to their providers.
If you have a family doctor but want to change to this Health Home, you first need to de-roster from your doctor then register with Health Care Connect.
