Are you living with a chronic condition, supporting a loved one, or just looking to stay active and informed? You’re not alone, and we’re here to help.

The Self-Management Program of Southeast Ontario offers free, practical health workshops that give you the tools and confidence to manage your health and well-being.

These workshops are also open to those at risk of developing a chronic disease, as well as family members and caregivers.
